Our staff are duty bound to respect your privacy, dignity and confidentiality and to treat you with courtesy at all times.
All patient information that we hold on paper or computer will be maintained securely and according to the regulations of the data protection act. You have the right to know what information we hold on you.
If you would like to see your records, please contact the practise manager to arrange an appointment.
We aim to give a friendly and professional service to our patients. If you are concerned about any aspect of your care please let us know. You can speak or write to the Practice Manager or your GP. Most problems can be quickly resolved, but if you feel we have not properly dealt with the problem, you can contact the Complaints Manager at Leicester City West PCT (details below). We welcome your suggestions on improvements to the service we provide.
The Parliamentary and Health Service Ombudsman website contains detailed information on raising a complaint about any aspect of the NHS in England. There is also a leaflet explaining the procedure of bringing a complaint to the ombudsman.

Here at Hockley Farm Medical Practice we are currently working on a Zero Tolerance policy. This means abusive behaviour of any kind will not be tolerated.
All episodes are documented onto patients medical records and will be brought to the attention of the Doctors and Management which could result in removal from the practice.
Please remember that the staff are here to help and are following procedures according to those set by the Doctors and Management.
Hockley Farm Medical Practice is committed to providing a safe, comfortable environment where patients and staff can be confident that best practice is being followed at all times and the safety of everyone is of paramount importance.
All patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. This chaperone may be a family member or friend. On occasions you may prefer a formal chaperone to be present, i.e. a trained member of staff.
Wherever possible we would ask you to make this request at the time of booking appointment so that arrangements can be made and your appointment is not delayed in any way. Where this is not possible we will endeavour to provide a formal chaperone at the time of request. However occasionally it may be necessary to reschedule your appointment.
Your healthcare professional may also require a chaperone to be present for certain consultations in accordance with our chaperone policy.
