Repeat Prescriptions:

If you take regular medication you will be required to have a MEDICATION REVIEW with the doctor.  When your review is due you will be contacted by the Surgery to arrange an appointment either face to face or over the telephone. FAILURE to keep this appointment will delay your medication.

If you have requested an item you have not had in the past 6 months it will take longer to process AND MAY BE REFUSED

To request a repeat using your paper repeat request tick all of the medication you need on the repeat slip on the back of your prescription or write clearly what you need on a piece of paper with your name and date of birth.

Unfortunately we are unable to take prescription requests over the phone.

Remember - we are closed bank holidays but we now open on Thursday afternoons for prescription collections.
